General Information Needed
  • Beginning and end dates of activity period to be analyzed  (MM/DD/YY)
  • Feeder Groups for Promotions  (You will be working with your consultant to determine appropriate Feeder Groups)
Required for Applicants, Hires, Promotions and Terminations
  • Name  (Last, First)
  • Gender  (Male/Female)
  • Race  (White, Black, Asian, Native American, Hispanic, Indian Subcontinent, or Hawaii Native)
  • Job Group (See Job Groups)
  • Date of Transaction (Application, Date of Hire, Date of Promotion, Date of Termination)
Required for Promotion Analyses only
  • Promotion from what job group?  (See Job Groups)
  • Promoted into what job group?  (See Job Groups)
Required for Termination Analyses only
