  • Employee Name  (Last, First)

  • ZIP Code/Address  (Employee's zip or city of residence (Include State also if outside of State where site is located)

  • Employee Number  (or Social Security Number)

  • Gender  (Male/Female)

  • Race  (White, Black, Asian, Native American, Hispanic, Indian Subcontinent, or Hawaii Native)

  • Job Title (Include such designations as “Senior” or LeveDesignation (e.g., Engineer V, Technician III)

  • Job Code  (If used)

  • Occupation Cod (If known)

  • Full-time or Part-time

  • EEO Classification  (Official/Manager, Professional, Sales, Technician, Office and Clerical, Skilled Production,  Semiskilled  Production, Laborer, or Service Worker)   

  • Job Group  (If known or See Job Groups)

  • Department Name  (Required)

  • Department Number  (If available (Not Cost Center Number)

  • Hire Date  (MM, DD, YY)

  • Birth Date (MM, DD, YY)

  • Salary (Either all annual or all hourly - do not include commissions, bonuses or other financial information)-

  • Salary Grade  (If used)

  • Salary Low  (If grades are used, this is the low end of the pay grade)

  • Location Name (If multiple sites and plans)

  • Location Number (If used)

  • Immediate Supervisor  (Supervisor, Manager, Director, Vice President, President (note:  immediate supervisor above each employee except top)
